This is a cast & crew gift version (production made and designed by Steve Horch) Star Trek: The Original Series style communicator from the Star Trek:Deep Space Nine episode 'Trials & Tribble-ations'.
An unknown number of these gifts were made that had both lights and sound (chirp) when opened. This one is made like the screen-used version and simply lights up. Three, that were virtually identical to this one, were used on-screen. Steve Horch used a cut-down vintage Sterling Plastics pencil box to make the buck for this prop.
This was acquired from HMS Studios during the Steve Horch estate sale.
A couple screen-caps of fromt he episode are included for reference.
