This is the uniform name tag patch worn by the character Chief Manilow Crocker played by Royce D. Applegate. This name patch was part of the uniform style worn by all Seaquest's crew during all 3 season of the show. The patch lists the character's name and the vessel the character is serving on, in this case the character is M. Crocker who is serving on the vessel DSV 4600. This name patches are located on the on the right hand chest of the uniform and are attached via a Velcro strip on the back of the patch to the uniform.
This particular name patch is specific to one episode only, which is the first season pilot episode titled "To Be Or Not To Be". This is because the only name patches to not have the wearers rank present in the name patch was during the filming of the pilot episode. After that the name patches were redesigned to incorporate the rank of the character in the name patch.
