This is Captain Archer's distressed uniform from the episode "Storm Front". It has been screen matched due to the rips on the back of the uniform, the bloodstains on the front, the hole/blood on the sleeve and many more numerous rips/bloodstains on the uniform. Worn by Scott Bakula as Captain Archer.
