This is a production used gauze pad from Amanda's "test" in Saw III. Amanda, played by Shawnee Smith, opened a stack of these after cutting her thigh in her flashback test from "Jigsaw." This one is covered in production blood, and came included with a certificate of authenticity from Raybin Management!!!!
