This prop wall sign decoration is seen in the TNG episode "The Quality of Life". When they are in the lab on Tyrus VIIa you can clearly see the sign in the entrance on top of the doorway. Said to be one of two made for the episode. 5/8" thick foam board. Second picture shows sign next to red square.
