This is the original bald cap make-up appliance, with attached cheek scar, that Sebastian Shaw wore for the climatic reveal moment in Return of the Jedi.  The piece is mounted on the original production used lifecast of Shaw, which was used in creating the baldcap.

The reveal was a one-day shoot, and this was the only appliance used.  It can be screen matched to the film and publicity shots.
