Doctor McCoy has several variations of this instrument, also known as an "anabolic protoplaser", on his surgical tray.  This is the largest of the 3 types, and the only version actually seen in use by McCoy onscreen, as in first season episode The Enemy Within, where he heals a crewman's hand injury with it. 

Red enameled resin body, textured base material similar to kydex, black resin nozzle cast from an original 60s Electrolux vacuum cleaner crevice tool, and photo-etched metal mesh nozzle tip.  Working dorsal trigger activates sound, vibration, and/or 2 different light effects, depending on mode selected.
