In fourth season episode An Evil Within, groups of Philadelphia residents suddenly find themselves fighting off attacks by tentacled eldritch monsters, only to find after the battles end that the nightmare creatures were really just nonthreatening human beings, and not monsters at all. 

After one man is brutally beaten and a second killed due to these apparent mass hallucinations, Pete and Myka investigate these police sketches created from eyewitness descriptions, with those involved seeing the first attacker (at a diner) as an impossible beast of reddish hue, and the second (at a dentist's office) as a similarly improbable multi-eyed creature of sickly green.
