This is Lindsay's Horror House waiver she signs in the halloween episode of season 2, where each of the main characters have to sign their own waiver. This one features an image of Lindsay dressed in a sailors outfit, and lists her full name and a medical condition of c**kaholic. The second page is identical to the first but lists a fake phone number. The polaroid image has a very slight tearing on the right, which occured due to how the item was shipped.
