This was purchased in March of 2009 from a gentleman who says it is part of a lot that he acquired directly from the 'Museum of Mom and Pop Culture': http://museumofmomandpopculture.com
The seller kept the original COA and made a copy of it for each of the two purchases I made. He notated the copies.
